چکیده

Abstract Background There is no biomarker to guide first-line of biotherapy in ulcerative colitis (UC). The main objective this multicenter pilot study was demonstrate the feasibility identifying a4b7 and/or TNF-a expressing cells by dual-band confocal endomicroscopy (CLE) into mucosa predict response vedolizumab as biotherapy. NCT02878083. Methods Patients with moderate severe UC, naïve were prospectively included and received vedolizumab. evaluated clinically, biologically, endoscopically histologically at week 22. Nonresponding patients subsequently treated adalimumab 30. Prior initiation vedolizumab, during endoscopy, 8 biopsies taken for extemporaneous ex vivo analysis CLE, histology microscopy. Blood samples collected before each infusion used immune (LT, LB, NK, monocytes) FACS trough levels. Use of confocal laser endomicroscopy to predict relapse of ulcerative colitis

BACKGROUND Assessment of inflammatory activity in patients with ulcerative colitis (UC) is crucial to the prediction of relapse. Confocal laser endomicroscopy (CLE) is an accurate tool for assessing inflammatory activity in UC patients. This study aimed to evaluate whether CLE could be used to predict UC relapse reliably. METHODS In total, forty-three patients with documented UC were analyzed...

Confocal laser endomicroscopy for the diagnosis of diversion colitis.

Confocal laser endomicroscopy (CLE) and virtual chromoendoscopy (VCE) are advanced endoscopic imaging techniques. i-Scan (Pentax, Japan) is a contrast-enhancing computed program [1], while CLE enables in vivo histological examination [2,3]. Studies from our group described the diagnostic utility of CLE and VCE, such as for Crohn’s disease [4–6]. However, data regarding the applicability of VCE ...

Cost-effectiveness of adalimumab, infliximab or vedolizumab as first-line biological therapy in moderate-to-severe ulcerative colitis

BACKGROUND There are no head-to-head randomised controlled trials (RCTs) comparing the effectiveness of biologics in ulcerative colitis (UC). We aimed to assess the cost-effectiveness of adalimumab, infliximab and vedolizumab as first-line agents to induce clinical remission and mucosal healing (MH) in UC. METHODS We constructed a decision tree based on a payer's perspective in the USA to est...

Confocal Laser Endomicroscopy

Confocal laser endomicroscopy (CLE) is a newly developed endoscopic technique that enables imaging of the mucosal layer during endoscopy at a subcellular level of resolution. The method can therefore be used for the assessment of changes in vascular architecture, connective tissue, and cellular components in the mucosa, enabling endoscopists to collect real-time in vivo histological images or "...
